Welcome to Parents' Foundation for Transitional Living

Thank you for your interest in Parents’ Foundation for Transitional Living. Whether you are here to learn more about a new home for you, a loved one or client, have an interest in joining our staff or would like to support our mission, we are happy you are here. Parents’ Foundation for Transitional Living, located in New Haven, Connecticut, is home to 27 residents, living with psychiatric conditions, who are dedicated to building skills and gaining back independence in order to live happy and fulfilling lives. We also provide case management and medication monitoring services to clients living independently, providing a safety net and support network needed to continue building healthy habits and confidence.

Mission

Our mission is to provide transitional, long-term, permanent and supportive housing for persons with chronic psychiatric conditions; promote a greater understanding of mental health conditions, and to provide educational opportunities for mental health professionals. We take this to heart in all that we do each day and strive to meet the needs of all residents and program participants through a customized approach to case management, goal planning, and skill development. No two people are alike which is why each member of our program community is cared for as an individual, with their own needs and goals at the center of every plan.
