What to Expect During the Admission Process at Parents’ Foundation for Transitional Living

Deciding to pursue residential support is a meaningful step, and at Parents’ Foundation for Transitional Living, we honor that by ensuring our admission process is thoughtful, individualized, and paced according to each person’s unique needs.


We understand that no two journeys are the same. That’s why our admission process is not rushed or one-size-fits-all. Instead, it is collaborative, transparent, and designed to ensure the best possible fit for everyone involved.


A Process Centered on the Individual

At Parents’ Foundation, the admission process is paced as needed for the individual and their support system. Some families move quickly through the process, while others may need more time to gather information, coordinate care, or simply feel confident in the next step.


Our goal is not just placement, it’s long-term success.


We work closely with:

  • Individuals

  • Families and guardians

  • Referring providers

  • Clinical teams and programs


From the first inquiry, we begin building a shared understanding of the individual’s strengths, goals, clinical needs, and readiness for transitional living.


Coordinating Clinical Care Before Move-In

One of the most important parts of our admission process is ensuring that clinical care is fully coordinated prior to move-in.


Because Parents’ Foundation is a supportive transitional living community, residents must have appropriate clinical providers established in the community. This may include:

  • Individual therapists

  • Psychiatrists or medication providers

  • Other outpatient supports


We believe stability and continuity of care are essential. Having services coordinated in advance allows residents to transition smoothly into our community without gaps in treatment.


Our team works collaboratively with families and providers to ensure these supports are confirmed before a move-in date is finalized.


The Admission Packet

To help us understand each individual holistically, we ask that an admission packet be completed. This typically includes:

  • Clinical history and current diagnoses

  • Medication information

  • Treatment history

  • Emergency contacts

  • Insurance information

  • Releases of information

  • Educational or vocational background


This documentation allows our team to thoughtfully review whether our program is clinically appropriate and supportive of the individual’s goals.


We take this review seriously because fit matters.


The Interview Process

Once the admission packet has been reviewed, we schedule an interview.

Interviews may take place:

  • In person, or

  • Virtually (depending on location and accessibility)


The interview is an opportunity for everyone involved to connect beyond paperwork. It allows us to:

  • Learn more about the individual’s goals and readiness

  • Answer questions about daily life in our community

  • Discuss expectations and structure

  • Explore how our program can support growth


Just as importantly, this is a chance for the individual and family to assess whether Parents’ Foundation feels like the right environment. We do offer a tour of the facility as well.

Admission is not just about whether someone qualifies; it’s about whether it feels like the best fit.


Ensuring the Right Fit

We believe strongly that successful transitions begin with alignment.

Before offering admission, we want to ensure that:

  • The individual feels comfortable and ready

  • The family feels confident and informed

  • The referring provider supports the placement

  • Our clinical and residential teams believe we can appropriately meet the individual’s needs


When everyone feels aligned, residents are positioned for stronger outcomes, greater independence, and meaningful growth.


Moving Forward

If admission is approved and all clinical coordination is complete, we work with families to determine a move-in timeline that supports a smooth transition.


From there, our team continues to provide structure, guidance, and support as each resident begins their journey toward greater independence.
