Care Coordination has been a cornerstone of success at Parents’ Foundation for Transitional Living. Many of our residents arrive after making meaningful progress in inpatient or clinical environments, where structured support, advocacy, and daily consistency have played a crucial role in stabilizing their mental health. But once those supports are removed—or even just reduced—maintaining that stability can be a challenge.

That’s why our structured approach at Parents’ Foundation is carefully designed to teach, support, and sustain the stabilization and consistency our residents need to continue thriving—this time outside of a clinical setting. We bridge the gap between intensive treatment and independent living by creating a safe, empowering environment where growth is both supported and celebrated.


A Holistic and Person-Centered Approach

Our Care Coordination support isn’t one-size-fits-all. It’s a holistic, person-centered approach to managing residents’ medical, psychiatric, and personal development goals. Care coordinators work closely with residents, their families (when appropriate), and external providers to ensure that every piece of the puzzle is considered—from medications to mindfulness, from doctor visits to dreams.

Here’s how our care coordinators support success every day:

  • Appointment Management

    We help residents schedule, prepare for, and attend medical and psychiatric appointments. Coordinators also assist with transportation logistics, so no resident has to face a critical healthcare moment alone or unsupported.

  • Advocacy

    Our staff act as liaisons between residents and external providers, ensuring that medical, psychiatric, and social needs are clearly communicated and adequately addressed. This kind of hands-on advocacy builds confidence and ensures residents feel heard and respected in every aspect of their care.

  • Goal Planning

    Together with residents, care coordinators develop individualized plans that reflect personal values, long-term goals, and recovery milestones. Whether it’s returning to school, building social skills, managing a chronic condition, or simply maintaining a daily routine, we believe each goal matters—and every success counts.
